Appointment requests and emergencies
A form submission is a request for follow-up. It does not reserve an appointment, guarantee availability, or confirm that a message has been reviewed. An appointment is confirmed only after the practice contacts you.
Website forms and email are not continuously monitored and must not be used for emergencies. Call the office for urgent dental concerns. For a life-threatening emergency, call 911.
Who may submit a request
Online requests should be submitted by someone aged 18 or older. A parent, guardian, or other authorized representative may contact us for another person. Provide accurate contact information and only information you are authorized to share.

Providing a number in a general inquiry form is not, by itself, an enrollment in marketing texts. Consent to marketing texts is not required to request care.
